1. ANKLE EXERCISE BOARD (MODEL SW38)

   Ankle exercise board. Rocker balance board design. Top board is mounted at center point on cylindrical rocker. Top size is 13 inches wide by 18 inches long, rocker has 3 1/2 inch diameter, overall height 4 1/4 inches. Top is covered by nonskid tread.

2. BACK BOARD

   Back board available in 3 models. Full length model is 16 inches wide by 71 inches long, has 2 plastic struts for strength and rigidity, includes 5 foot chest and waist straps and 36 inch ankle strap. Extraction board model is 16 inches wide by 32 inches long and includes 5 foot long chest and waist straps. Infant back board model is 14 inches wide by 37 inches long. Used for managing spinal injuries. Straps are 2 inch wide nylon with quick release buckles. Options: cervical immobilization device and adult sized head block.

3. BARIATRIC HI-LO TREATMENT TABLE (MODEL 4795)

   The Bariatric Hi-Lo Treatment Table, model 4795, is a powered treatment table designed for use by individuals with mobility disabilities or spinal cord injury. This treatment table has motorized height adjustment from wheelchair level, and a power backrest that adjusts from 0 to 75 degrees. Lifting/lowering is provided by two synchronized direct-drive lifts with hand controls. The table has a laminate frame and an extra-wide tabletop with a high-density urethane foam cushion and vinyl upholstery. OPTIONS: Digital scale (model 595). DIMENSIONS (HxLxW): 20 to 29 x 78 x 32 inches. The cushion is two inches thick. CAPACITY: 600 pounds.    The Bariatric Tilt Table, model 6045-709, is a powered tilt table designed for use in physical therapy. A heavy-duty electric motor controls the table's angle of tilt from 0 to 90 degrees. The table has a tilt indicator display and can be operated from either side with hand controls. It has a heavy-duty steel frame, vinyl upholstery, thick high-density foam padding, three extra-long safety straps, and locking casters. POWER: Uses 110V. DIMENSIONS (HxLxW): 33 x 77 x 34 inches. 5. BARIATRIC TREATMENT TABLE (MODEL 4086)

   The Bariatric Treatment Table, model 4086, is a treatment table designed for use in physical therapy. This bariatric treatment table has six high-pressure laminate legs, a plywood sub-top, and a full-width laminate shelf. The table top is cushioned with high-density urethane foam with vinyl upholstery. DIMENSIONS (HxLxW): 26 x 78 x 30 inches. Clearance under the shelf is 4 inches. The foam padding is 2 inches thick. COLORS: Vinyl upholstery is available in black, burgundy, deep wine, dove gray, forest green, gray, grotto green, navy blue, Nordic blue, oak brown, proform black, rodeo tan, rose, or slate blue.

6. CUBEX THERAPY SYSTEM ON WHEELS

   The Cubex Therapy System on Wheels is a fine motor training kit designed for use with individuals with neurological disabilities and/or who use wheelchairs. This wheeled station accommodates four users standing and/or in wheelchairs At the ends of the unit are 90-degree fold-away work surfaces with bolt boards and a pegboard that opens to 60 or 90 degrees. Other features include a slide-out full-width plastic-laminate work surface and a reversible activity board in the top with a pegboard on one side and a smooth easel on the other. The board rotates 360 degrees and locks flat or at a 60-degree angle. The unit also includes a locking double-door cabinet with four storage tubs and two shelves. The cart is equipped with heavy-duty swiveling, locking casters. Included with the system are 100 plastic pegs, 24 bolts, and five tools. DIMENSIONS (HxLxD): 38 x 38 x 24 inches. The end work surfaces adjust from 27 to 37 inches high. The casters are four inches in diameter.

7. ELECTRIC WORK TABLE (MODEL 4375)

   The Electric Work Table, model 4375, is a powered height adjustable work or therapy table designed for use by wheelchair users. This height-adjustable table has a direct-drive lift mechanism that elevates the tabletop and a pedestal design that allows wheelchair access, and a hand-control panel mounted on the underside of the tabletop. The table has a high-pressure laminate top with a natural oak finish and protective molding along the edges. It can can accommodates two wheelchairs and up to five ambulatory individuals. POWER: Uses 110 volts. DIMENSIONS (HxLxW): 27 to 43 x 60 x 36 inches. The tabletop elevates from 27 to 43 inches high.

8. EXTRA WIDE BARIATRIC TREATMENT TABLE (MODEL 4074)

   The Extra-Wide Bariatric Treatment Table, model 4074, is designed for use in physical therapy. This table has an extra-wide upholstered top with extra-thick, high-density urethane foam for bariatric use. The table is constructed from reinforced oak and has three storage cabinets, each with laminate interiors, and one adjustable shelf. The storage cabinet doors have concealed, self-closing Euro-hinges. OPTIONS: Built-in digital scale with 600 pound capacity (model 595). DIMENSIONS (LxW): 78 x 34 inches. The foam pad is 3 inches thick. CAPACITY: 800 pounds.

9. FOLDING PARALLEL BARS (MODEL 1319)

   Folding Parallel Bars, model 1319, are parallel bars designed for use by individuals with lower extremity or walking disabilities or who have had a stroke. The unit has heavy-gauge steel base plates, uprights, and fittings with a powder coated finish. The one-piece handrails are stainless steel. Each upright telescopes and locks into a choice of nine height positions with a fail-safe ball-tip locking pin. The unit folds for storage. DIMENSIONS (HxLxW): 28 to 41 inches x 9 feet x 25 inches. The unit is 10 inches wide when folded. The uprights telescope up to 13 inches. The handrails are 1.5 inches in diameter. CAPACITY: 400 pounds. COLORS: Black finish.

10. HEAVY DUTY TREATMENT TABLES

    Therapy treatment tables with wood frames and upholstered tops. Available in several 30 inch height models. Standard treatment table has optional H brace, upholstered shelf, or divided shelf and is available in 24, 27, or 30 inch widths and 72 or 78 inch lengths. All models have leg levelers. Legs and frames are solid oak, cabinets are oak plywood. Table tops have extra firm polyurethane foam cushion, upholstered with Uniroyal naugahyde. Optional accessories: drawers, upholstered arm board or pillow, paper holder and cutter, and 4 inch locking casters.

11. HI LO OT WORK TABLES

    Adjustable height occupational therapy work table. Available in hydraulic model with foot or hand operated actuator. Adjustable height from 29 to 41 inches, self brakes at any level. Top is 48 by 60 inch oak butcher block surface. Base is 32 inches long by 20 inches wide by 1 1/2 inches thick.

12. HIGH LOW TREATMENT TABLE

    Powered adjustable height treatment table with 30 inch raising back section, wooden base with one drawer, and storage shelf between pedestals. Foot pedal operated actuator raises and lowers table. Adjustable height 23 to 35 inches. Top section is 78 inches long and 27 to 30 inches wide. Made of solid oak and oak plywood. Table top has extra firm polyurethane foam cushion, upholstered with Uniroyal naugahyde. Options: upholstered arm board or pillow, and paper holder and cutter.

13. HI-LO MULTI-POSITION TABLE (MODEL 6052)

    The Hi-Lo Multi-Position Table, model 6052, is a powered treatment table designed for use in physical therapy. This height-adjustable tilt table may be used in a variety of treatments, including lateral flexion and postural drainage. The motorized height adjustment lowers the table for wheelchair access. The three-section top features nose cut-outs with plugs in both the head and leg/backrest sections. The head and leg/backrest sections have positive lock-in levers. The head section raises manually up to 80 degrees and lowers to 35 degrees below horizontal. The leg/backrest raises up to 80 degrees and lowers to 30 degrees below horizontal. This table is operated with foot controls. It has a heavy-duty steel frame, a vinyl upholstered top, retractable casters, and a "Failsafe" brake system. POWER: Uses 110 volts. 14. HI-LO TILT TABLE (MODEL 6058)

    The Hi-Lo Tilt Table, model 6058, is a powered tilt table designed for use by individuals with mobility disabilities or spinal cord injury. This wheelchair-accessible tilt table has an electric motor that lowers the table for transfer by wheelchair users. The table tilts from a horizontal position to a 90-degree angle at a fixed speed of 4 degrees per second. The padded top is upholstered in vinyl, and has three extra-wide safety straps with hook-and-loop closures. The table has a heavy-duty steel frame, swivel/lock casters, a foot control, and a tilt-angle indicator on both sides. POWER: Uses 110 volt current. DIMENSIONS (HxLxW): 19 to 34 x 74 x 24 inches. The casters are three inches in diameter. COLOR: The upholstery is gray.

15. MAT TABLE

    Mat tables with wood frames and upholstered tops, available in 3 models. Wall mounted model is attached to wall, stands on 2 legs that fold up for storage so that mat extends 8 inches from wall when locked into position, size 21 inches high by 60 inches wide by 84 inches long. Mat table with permanent mat is 21 inches high and available in 36, 48, 60, or 72 inch widths and 72, 84, or 96 inch lengths. Raised rim mat table model has separate mat, table is 19 inches high and available in 37, 49, 61, or 73 inch widths and 73, 85, or 97 inch lengths. Frames and legs are solid oak. Mats are made of high density urethane foam covered with nylon reinforced vinyl material.

16. MULTIPLE PLACE CUT OUT TABLE (MODEL 6254)

    Table with 4 cut outs to provide individualized table work space for 4 children. Height adjusts from 23 to 36 inches. 2 cutouts are 8 inches by 13 inches and 2 cutouts are 8 inches by 16 inches. Overall size is 96 inches long and 30 inches wide. Tabletop is heavy duty plastic. Useful for children in wheelchairs for sliding chairs close to table.

17. P T PULLEY FRAME (MODEL SW16)

    Pulley frame. Can be used with mat table, treatment table, wheelchair, or stationary chair. Includes 2 adjustable pulley mounts, additional mounts can be added. Solid oak with steel supports. Must be mounted to wall and ceiling. Dimensions 86 inches high by 36 inches wide by 84 inches long. Wall and ceiling mounting hardware is not provided. Large variety of pulley frame accessories available.

18. POWDER BOARD (MODEL SW24)

    Exercise board. Laminate top is 30 inches by 40 inches. Wooden legs are 7 inches high when extended and fold to 2 inches high.

    Exercise table or powder table with curved recess for close approach of user. Adjustable height from 22 to 38 inches. Laminate top is 36 inches wide by 60 inches.

20. RAMP AND CURB SET (MODEL SW18)

    Ramp and curb set. Inclined ramp has flat platform at top and is attached at 90 degree angle to 3 section pullout curb with one 2 inch, one 4 inch, one 6 inch high riser. Nonslip tread on all exercise surfaces. Overall dimensions 8 inches high by 44 inches wide by 87 inches long.

21. SCOOTER STOOL

    Aluminum stool mounted on ball-bearing casters. Tan padded seat. 18 inches high. Useful for helping teachers work at child's level.

22. SHOULDER FINGER LADDER (MODEL SW35)

    Shoulder Finger Ladder for active shoulder exercise. Has 36 steps at 1 1/4 inch intervals for finger placement. Size 2 1/4 inches wide by 54 inches long by 1 3/8 inches thick. Mounts on wall.

23. SINGLE STAND IN TABLE (MODEL 6101)

    Stand in table designed to assist children in maintaining standing position while giving them a table top area for work or play activities. Height is adjustable. Leg divider is provided which can be attached to standing platform to encourage abduction. Wood construction. Has a wide base to prevent tipping. Table top is stain proof and easily cleaned. Box size: 12 inches by 14 inches for large size. Small box size is 10 by 12 inches. Table top size for both models is 24 by 24 inches by 34 inches high.

24. SPACE SAVER MAT PLATFORM (MODELS 1420 & 1421)

    The Space Saver Mat Platform is a mat table designed for use by individuals with mobility disabilities or spinal cord injury. This wheelchair-accesible table has six legs and the top is padded with high-density urethane foam and upholstered in nylon-reinforced vinyl. All exterior surfaces have a laminate finish. DIMENSIONS (HxLxW): 18 inches x 4 x 7 feet (model 1420) or 18 inches x 5 x 7 feet (model 1421). The foam padding on both models is two inches thick. CAPACITY: 1,000 pounds. COLORS: The laminate is available in natural oak or folkstone gray and the top is available in blue or gray.

25. TRAINING STAIRS

    Therapy training stairs available in 3 models. Handrail enclosed platform model has four 6 inch risers. Straight platform model has two 6 inch and three 4 inch risers. Stairs are 30 inches wide, risers are 10 inches deep, platform is 30 inches wide by 24 inches deep. Has double hand rails on both sides. Oak with nonskid tread.

26. TRANSFER BOARDS

    Transfer board made of solid red oak electronically glued for strength and crack resistance. Finished boards are coated with urethane sealer and polished. Unfinished boards are sanded smooth and are ready to finish. Boards are 8 inches wide, available without hand hole, with one hand hole or with 2 hand holes, and in 24 or 30 inch lengths.

27. WHEELCHAIR TRAY (MODEL 5061) & WHEELCHAIR TRAY CHILD SIZE (MODEL 2467)

    Wheelchair tray with rim fits all size wheelchairs with full length or desk arms. Sides are tapered to permit chair propulsion with tray in position. Smooth plastic top. 24 by 19 inches with 15 by 6 inch cutout; child size 20 by 19 inches with 12 by 3 1/2 inch cutout.
