The tray includes a universal mounting system, continuous 5/8-inch high lip/retaining edge; telescoping support arms for strength and convenience; a rim that is fused into the plastic tray for added strength without added weight; and captive locking knobs. The tray is secured to the wheelchair arms by Poly-Lock hook and loop fasteners. Custom stomach and control box cutouts available. 10. HIGH VISIBILITY TRACTION TAPE (HVTT) (MODELS 7G & 7R)

    High Visibility Traction Tape (HVTT) is anti skid glow tape designed to increase visibility and reduce the risk of tripping for people with low vision or mobility disabilities. Model 7R is retro-reflective, and model 7G glows in the dark for up to ten hours of visibility in poor or no light conditions. These peel-and-stick tapes come in rolls and are designed for semi-permanent indoor or outdoor applications. Tape is available with medium texture (mineral coated) or fine texture without mineral coating (model 7G only). DIMENSIONS: Rolls are 3 or 6 inches wide. Custom widths up to 48 inches are available. COLOR: Model 7G is available in clear with a white stripe, black with a white stripe, yellow with a black and white stripe, red with diagonal white stripes, or clear with white dots. This unit is effective in teaching walking patterns, weight bearing and foot placement. Uses dual channel, light, and auditory feedback controls, and two 8-ounce sensitivity switches. POWER: Battery. WEIGHT: 1 pound.

12. NATURAL BIRCHWOOD TRAY (MODELS 11JA.1 & 11JB.1)

These trays are made of 9-ply birchwood with a clear lacquer finish and have a continuous 5/8 x 1/8-inch ABS plastic rim. Model 11JA.1 includes 2-inch wide adjustable Poly-Lock straps with D-rings for mounting; Model 11JB.1 does not include mounting straps. DIMENSIONS: 19 x 24 x 3/8 inches. 13. PEDAL LEVELING SYSTEM 6.7

This system is made for use with the Consumer Care Products Inc. tricycles (see separate entries). The device is made of nylon cord that runs through steel eyelets screwed into frame. WEIGHT: Approximately 1 pound.

14. POLY-LOCK FABRIC TAPE FASTENER

    --- DISCONTINUED. (Verified 10/2009) RETAINED IN DATABASE FOR REFERENCE. --- Poly-Lock is an industrial quality fabric hook and loop fastener. This tape fastener is made of 100 percent woven nylon fabric that is preshrunk to assure dimensional stability. The "loop" fastener surface is napped (evenly raised) for strength and reclosure life. This material is washable and dry cleanable, can be gas or steam sterilized or autoclaved with most common systems, resists attack by most common solvents and alkaline solutions (acid solutions, however, will cause deterioration of the fastener), and is water resistant (closure strength is reduced after prolonged exposure to water, but returns to full strength when dried). Poly-Lock fabric tape fasteners are available in several models of plain backed sew-on tape (standard flame resistant, extra flame resistant, and polyester hook and loop) and pressure sensitive backing tape (woven anti-plasticizer, woven high temperature acrylic, and polyester high temperature acrylic). METHODS OF ATTACHMENT: Poly-Lock tape can be attached to a variety of materials using the pressure sensitive adhesive backing, by sewing the tape with a manual or semi-automatic sewing maching, with ultrasonic bonding, or press bonding. USES: Include, but are not limited to, clothing and footwear; splints and braces; panels and doors, rehabilitation aids and wheelchairs; pictures and signs; carpets and matts; and control boxes or appliances. DIMENSIONS: Widths available range from 0.75 to 4 inches; sold in 25 to 50 foot rolls. COLOR: Black, white, beige, or custom colors (extra charge).

15. POSITIONING AID HIGH CHAIR (MODEL 17)

(Verified 10/2009) RETAINED IN DATABASE FOR REFERENCE. ---- The Positioning Aid High Chair, model 17, is a height adjustable, multipurpose, handcrafted chair specially designed around the needs of children 9 months to 5 years old with central nervous system problems. The chair is made of maple and includes an adjustable pummel, thoracic support, lumbar support, footrest, and positioning belts. The slip covers are made of washable and removable neoprene backed fabric. The base has 4 covered casters; 2 have brakes and 2 do not have brakes. The chair disassembles for transport and storage. CAPACITY: Maximum user weight is 68 pounds. DIMENSIONS (LxWxH): 17 x 24 x 19 to 31 inches. WEIGHT: Approximately 45 pounds.

16. RX WORKSTATIONS (MODEL 9E)

(Verified 11/09) RETAINED IN DATABASE FOR REFERENCE. --- The Rx WorkStations, model 9E, is a 3-piece modular workstation that consists of an Adjustable Height Table (9A.1), an Adult Slant Board (8B.2), and a Self-storing Side Table (9.9A). TABLE: This folding leg table has a natural maplewood top and solid hardwood frame with protective edging. Leg height is adjustable from 23.75 to 34.75 inches. SIDE TABLE: The pull out side table attaches to the table to form an L-shaped environment. It also has a maplewood top. SLANT BOARD: This slant board with stomach cutout has a machanical adjustment for an infinite number of angles. OPTIONS: Also available from manufacturer are fixtures for bringing materials and supplies to the user including articulating arms, swivel action and movable bases, apertures for computer paper and wire management, communication aids, drawers for pencils and keyboards, turn table, lamp, and stands for computers, phones, and or letters. Models 7.1.1 and 7.1.2 consist of extra bases, Poly-Lock hook and loop mounting pads with release pin and 1/4-20 threaded insert (7.1.1) or stud (7.1.2). Models 7.1.3 and 7.1.4 consist of the base only with release pin and 1/4-20 threaded insert (7.1.3) or stud (7.1.4). HOLDING STRENGTH: Approximately 20 to 50 pounds for all models. 18. TALKING SIGN / MOTION PAD (TSMP)

State-of-the-art microchips provide clear recording and long-term reliability. The unit also features a built-in chime as an alternate means of alert or communication. Other standard features include lock in/lock out record for security, ten-second message with aut playback, and a built-in hanger hole. Detailed instructions are included. 19. WHL ATTACHMENT HARDWARE (MODEL 10.1)

Each set contains four 1/4-20 bolts, four 1/4-20 hex locknuts, and four 1/4-20 self locking acorn nuts. WEIGHT: Approximately 1 pound.
