1. ADJUSTABLE J HOOK BRACKET HARDWARE SEATING PACKAGE (PACKAGE 20XX)

   The Adjustable J Hook Bracket Hardware Seating Package, package 20XX, is seating hardware designed for use on a wheelchair. The Package includes a solid back insert including T-nuts; a solid seat insert including T-nuts; a wide, 1.5-inch offset seat to back hinge; a pair of 1- inch diameter tube two-hole J hooks; a pair of five-hole adjustable L brackets; a pair of 1-inch diameter tube six-hole J hooks; a pair of two-hole adjustable L brackets; a 1-inch diameter tube drop stop; and a 1-inch diameter tube drop stop with rotating hold down. DIMENSIONS: 1-inch diameter hardware is standard.

2. ADJUSTABLE SPLIT SOLID SEAT INSERT (MODEL SSP80XX)

   The Adjustable Split Solid Seat Insert, model SSP80XX, is a solid seat insert designed for use on wheelchairs. The solid seat insert includes HR foam, T-nuts, and an adjustable split seat, model MOD4900. DIMENSIONS: The foam is 1.5 inches thick.

3. ANTI-THRUST SOLID SEAT INSERT (MODEL SSP40XX)

   The Anti-Thrust Solid Seat Insert, model SSP40XX, is a solid seat insert designed for use on wheelchairs. The solid seat insert includes HR foam, T-nuts, and an anti-thrust seat, model MOD4400. DIMENSIONS: The foam is 1.5-inches thick.

4. J2 DEEP CONTOUR CUSHION

   The J2 Deep Contour Cushion is a gel and foam seat cushion designed for use in wheelchairs by individuals at high risk for skin breakdown. This cushion features a foam base support structure and a longer seat well that narrows toward the rear to accommodate a posterior pelvic position. A Jay Flow Fluid Tri-pad tops the foam base and has a three-dimensional design for superior pressure distribution and to prevent bottoming out. A thick foam insert in the fluid bladder assists in pressure distribution and provides a buffer between the cushion base and the fluid pad. This cushion is available with the choice of a standard stretch cover, an air exchange cover, or an incontinence resistant cover.

5. JAY ACTIVE BACK (MODELS 520, 522, 525, 527 & 528)

   The Jay Active Back, models 520/522/525/527 & 528, is a back support cushion designed to provide comfort and stability for chair users. This contoured support cushion is composed of lightweight materials and features adjustable height and back angles designed to help relieve back pain. It also includes an "Air Exchange" cover designed to reduce perspiration and heat buildup. With a rigid back shell, this support cushion is not intended to bow or stretch. DIMENSIONS: All models feature adjustable heights from 12 to 16 inches. Widths - Models 520 (18 inches); 522 (17 inches); 525 (16 inches); 527 (15 inches); 528 (14 inches). WEIGHT: 1.7 to 2.7 pounds.

6. JAY ACTIVE CUSHION

   The Jay Active Cushion is a gel and foam flotation seat cushion with adductor wedges designed to prevent leaning, slouching and pressure sores, and to encourage correct posture. The cushion has a contoured foam base with a Flolite pad. Flotlite is a liquid that conforms to the user's body and does not return to its original shape; this feature protects the skin from pressure sores. The fluid is encased by three top and two bottom layers of urethane with a center seam that prevents the Flolite from moving from side to side so leaning is prevented and scoliosis is discouraged. The cushion has a machine-washable AirExchange cover with a non-skid bottom and Velcro attachments. The cover also has a concealed pocket. An incontinence cover may be purchased on these cushions in lieu of the standard AirExchange cover for the same price.) SIZES: For adults only; Regular Short, Regular, Regular Long, Large, Narrow Short, Narrow, Narrow Adult Long, and Extra Narrow Adult Short. DIMENSIONS: 18 x 16, 18, or 20 inches; 20 x 18 inches; 16 x 16, 18, or 20 inches; and 14 x 16 inches.

7. JAY ADDUCTOR WEDGES & ABDUCTOR WEDGES / BUILD-UPS

   The Jay Abductor and Adductor Wedges are accessories for the Jay Cushion. The firm foam wedges attach to the seat cushion with velcro or glues to separate (abduct) the legs, or position them closer together (adduct). DIMENSIONS: Adductor wedges are available in 1-inch (no. B119J, $25/pair) or 2-inch (B113J, $30/pair) sizes for adults, and a 2-inch junior/child (B163, $30/pair) size; Abductor wedges are 2 inches for adults (B118W, $25); abductor build-ups are also 2 inches (B118 and B168 Child/Junior, $15 each).

8. JAY ADJUSTABLE SOLID SEAT

   The Jay Adjustable Solid Seat is a solid seat insert designed to improve posture by providing a firm, fully-adjustable base of support. The seat is adjustable in height, width, depth, and angle to allow an immediate fit and future change, when needed. The seat is made of rib-reinforced urethane and has an anti-slide surface so that cushions will not shift. The seat insert is designed to fit all wheelchairs; the universal cross-bar cut-out allows dropping the seat depth on virtually all wheelchair brands. The seat attaches to the wheelchair's rails with a spring steel hook that slides on and snaps out, a hook retainer to prevent the seat from accidentally popping out of the wheelchair, and a hook stopper to prevent the seat from sliding forward. The seat angle is adjusted with hooks at different heights for front-to-back or side-to-side seat angle. The insert has 2.5 inches of height adjustment to raise or lower the seat in 1/2-inch increments from 1-inch above to 1.5 inches below the wheelchair rails. SIZES: Adult (no. 270) adjusts from 15 to 18 inches wide and 12 to 18 inches long; Large Adult (no. 273, $200) adjusts from 18.5 to 22 inches wide and 12 to 20 inches long; and Small (no. 272) adjusts from 11.5 to 14.5 inches wide, and 10 to 16 inches long.

9. JAY BACK CUSHION

   The Jay Back is a foam and gel back support cushion designed to protect against scoliosis, kyphosis, instability, back pain, and skin breakdown. The cushion consists of an impact resistant ABS plastic shell with a middle layer of molded, contoured foam, and a Flolite spinal insert to prevent pressure problems on the spine. Flolite is a gel-like liquid that will conform to the user's body, but resists returning to its original position to prevent pressure on bony prominences. The water-resistant foam layer can be Velcro-detached from the shell for washing or for making custom cutouts to accommodate minor deformities. The back height of the shell is adjustable from 15 to 22 inches. The cushion has lateral trunk supports that attach under the cover with Velcro. These supports can be adjusted up or down, in or out, to support against side to side leaning and curvature of the spine. An adjustable lumbar suppor moves up or down behind the Flolite spinal insert to help restore lumbar curve and to reduce back pain by increasing sitting stability. All components are housed in a removable, washable AirExchange cover lined with 97 percent air foam to provide air circulation and relieve sweating. COMPATIBILITY: The Jay Back will fit virtually any detachable arm wheelchairs; the cushion can be used with the Jay, Jay Active, or Combi (see separate entries); and the Jay Back is compatible with the Otto Bock head rests (see separate entry). SIZES: Models are available to fit wheelchairs with widths of 9 to 20 inches. Lateral supports come in Regular, Large, and Extra Large.

10. JAY COLORED / PERSONALIZED COVERS

    The Jay Colored/Personalized Covers are cushion covers available for all Jay products in all styles as a special order item. The covers are made of 100 percent polyester, and are machine washable and dryable. Personalized covers have the customer's name embroidered on the label (maximum 17 letters); there is a $25 surcharge for personalization on colored covers, and an $18 surcharge for personalization on the standard black cover. COLOR: Color choices are Fuchsia Purple, Hot Pink, Fire Engine Red, and Royal Blue. Sample swatches are available through Customer Service. There is a $21 surcharge for colored covers, with the exception of Jay Cushion and Jay Back covers for children (in 10, 12, and 14-inch widths) which are stocked in color at no surcharge. DELIVERY TIME: Allow 5 extra working days for production time on personalized items.

11. JAY COMBI POSTURE SEATING SYSTEM

    The Jay Combi Posture Seating System consists of two foam cushions, one contoured, dual density seat cushion and one lumbar support back cushion, designed to reduce sitting fatigue, lower back pain, and slumping while promoting correct posture. COMBI SEAT CUSHION: The Combi seat is molded from two different kinds of foam. Inside is a contoured foam core that is firm, for support. Outside, a second layer of soft foam is added for comfort. The foundation resists compression from leaning to help prevent sliding forward into a slumped posture. The molded foam is rolled at the edges to reduce leg fatigue, and forms a leg separator to help position legs properly. The cushion bottom has a non-skid bottom to eliminate the cushion from sliding in a chair. BACKREST: The Combi backrest is designed to provide support to the lumbar region to restore the natural lumbar curve and reduce fatigue to the lower back. A removable foam insert allows adjustment of lumbar support to suit individual needs. The back cushion has a breathable, stain resistant Visa polyester cover that unzips for washing. The backrest can be adjusted up or down on the seat with Velcro attachments to accommodate a variety of body lengths. SIZES: For adults ($110 except large), Regular Short, Regular, Regular Long, Large ($135), Narrow Short, Narrow, and Narrow Long; Junior and Child sizes are also available ($90 each). DIMENSIONS: 18 x 16 inches, 18 x 18 inches, 20 x 18 inches, 15.5 x 16 or 18 inches, 16 x 20 inches for adult sizes; 14 x 14 inches or 12 x 12 inches for child sizes. WEIGHT: Less than 25 ounces. COLOR: Burgundy or navy pindot pattern.

12. JAY MODULAR BACK

    The Jay Modular Back is a solid back insert made of aluminum that will accommodate either a Jay Custom Contoured Insert to fit deformities, or a Jay Standard Insert. The aluminum shell includes recline and depth adjustment, foam side guards, and clamp-on hardware. The Custom Contoured Insert includes a foam back with shim, full-width fluid pad, and an AirExchange cover. The Standard Insert includes a foam back with a spinal fluid pad and AirExchange cover. DIMENSIONS: Back shell is available in models to fit wheelchairs 14 to 18 inches wide in one inch increments ($350 each) and 20-inch wide wheelchairs ($420); the Custom Contoured Insert will fit wheelchairs with a width of 14 to 18 inches ($400 each), or 20 inches ($475); the Standard Insert will fit wheelchairs with a width of 14 to 18 inches ($200), or 20 inches ($240).

13. JAY PROTECTOR CUSHION

    The Jay Protector Cushion is a gel and foam seat cushion designed to temporarily (up to 4 hours) protect the skin against pressure sores in any situation - in the shower, during recreation activities away from a wheelchair, while working on the floor, etc. The cushion has a sling design with waist and leg attachments that maintains the cushion in place during transfers. The cushion has a concealed molded foam base that absorbs impact and assists in stable sitting. On top of the foam core is a Flolite pad. Flolite is a liquid material designed to conform to the user's body and not return to its original shape, thereby reducing pressure to the bony prominences. The foam core and Flolite pad are covered with urethane to seal out water for bath or boating use. The cushion is encased in a removable, washable cover with a non-skid bottom. The sling attachment serves several purposes: to secure the cushion to the body so it won't move when the user moves; to protect the coccyx and hip bones with a 3/8-inch water-repellent foam lining; and to provide low back support for stable sitting on the floor. The sling's bottom is made of nylon pack cloth. SIZES: Adult (no. 140, $195) or Junior (no. 143, $175). DIMENSIONS: Adult size is 14 inches wide by 9 inches deep; Junior size is 11 x 7 inches.

14. JAY SCOOTER SEATING SYSTEM

    The Jay Scooter Seating System is a portable seating system for powered scooters designed to reduce fatigue and back pain, and to provide more support and stability. The system consists of a Jay Cushion with Quadrant Pad ($385) and a Jay Back ($350). The Jay seat cushion's quadrant pad is divided into four sections to support against leaning and sliding, and is filled with Flolite. Flolite is a gel-like liquid designed to conform to the user's body and to retain the user's form to maximize relief of pressure and shearing. The Flolite quadrant pad is fitted on a urine-resistant foam core base shaped to encourage a stable, upright posture. The Jay Back features a set of Velcro-adjustable lateral supports that help the user sit straighter. The back is adjustable in back angle and lumbar support to help combat the probelms of curvature of the spine, instability, and back pain. Both the Jay seat cushion and back cushion are molded to fit the "Tempress" boat-style seat available on most scooter models. Both cushions have an AirExchange cover. DIMENSIONS: Seat cushion (no. 112) is 17 x 18 inches; seat back (no. 512L) is 17 x 17 inches.

15. JAY STANDARD WHEELCHAIR CUSHION

    The Jay Standard Wheelchair Cushion is a gel flotation seat cushion designed to prevent and heal pressure sores and improve posture. This cushion features a urethane foam base that is contoured to reduce pressure on the ischials and the coccyx, and to encourage upright posture by discouraging leaning and slumping to one side. On top of the foam base is a Flolite pad. Flolite is a fluid that, once conformed to the user's body, remains in place rather than returning to its original shape to reduce pressure. Both of these components are encased in a breathable stretch cover. SIZES: For adults, Narrow Short, Narrow, Regular Short, Regular, and Extra Narrow Short; also Junior, Child, and Small Child. DIMENSIONS: 18 x 16 inches, 18 x 18 inches, 15.5 x 16 inches, 15.5 x 18 inches, 14 x 16 inches for adult sizes; 14 x 13 inches, 12 x 11 inches, and 10 x 10 inches for children and junior sizes.

16. OTTO BOCK TALL BACK AND MODULAR BACK HEADREST MOUNTING KIT & OTTO BOCK HEADREST ADAPTER KIT

    The Otto Bock Tall Back and Modular Back Headrest Mounting Kit ($5) and the Otto Bock Headrest Adapter Kit ($20) are headrest accessories that allow attachment of Otto Bock headrest mounting hardware directly to the Jay Back cushion, compensating for the contour of the cushion.
