Picture of WHO'S SHOES ID KIT Who's Shoes ID Kit is an emergency medical identification tag designed for use by children and adults with cognitive or communication disabilities or Alzheimer's disease. Each kit includes two identification bands that can be fastened to most shoe styles. The band is made of Velcro-brand hook-and-loop fastener and includes a water-resistant information label. The information label includes space for the individual's name, parent or caregiver name and emergency phone numbers, physician's name and phone number, medical conditions, and the individual's date of birth. A separate expanded emergency card is also included. DIMENSIONS (LxW): 2 x 0.75 inches folded and 6 x 0.75 inches unfolded. COLOR: Neon pink or royal blue.
